Cookie Policy

PromiseCare Medical Group
Effective Date: March 12, 2026
Last Updated: March 12, 2026

This Cookie Policy explains how PromiseCare Medical Group (“PromiseCare,” “we,” “us,” or “our”) uses cookies and similar tracking technologies on our websites at promisecare.com, promisecaremanagementservices.com, hemetcommunitymedicalgroup.com, and all affiliated PromiseCare websites (collectively, the “Sites”).

This policy should be read alongside our Privacy Policy and our HIPAA Notice of Privacy Practices.

Important: The cookies and tracking technologies described in this policy operate on the public-facing portions of our Sites. They are not used within the secure Patient Portal. No protected health information (PHI) is collected through the cookies described in this policy.

What Are Cookies?

Cookies are small text files that are placed on your device (computer, tablet, or mobile phone) when you visit a website. They are widely used to make websites work, work more efficiently, and to provide information to website operators about how visitors use their sites.

Cookies can be:

  • Session cookies — temporary files deleted when you close your browser
  • Persistent cookies — files that remain on your device for a set period or until you delete them
  • First-party cookies — set directly by PromiseCare
  • Third-party cookies — set by external services we use (such as Google Analytics)

Cookies We Use

1. Strictly Necessary Cookies

What they do: These cookies are essential for the Sites to function. They enable core features such as page navigation, form submission, and access to secure areas of the Sites. Without these cookies, services you have requested cannot be provided.

Can you opt out? No. Because these cookies are strictly necessary for the Sites to operate, they cannot be disabled.

Examples of what they do:

  • Maintain your session as you navigate between pages
  • Remember your form progress
  • Ensure secure access to the Patient Portal login page

2. Analytics and Performance Cookies

What they do: We use Google Analytics to collect information about how visitors use our Sites. This includes which pages are visited most often, how long visitors spend on each page, where visitors come from (referral source), and how visitors navigate through the site. This information helps us improve the Sites and the experience we provide.

Who sets them: Google LLC (third-party)

Data collected: Anonymous/pseudonymous usage data including IP address (anonymized), browser type, device type, pages visited, session duration, and referral source. Google Analytics does not collect PHI through our Sites.

Google’s Privacy Policy: https://policies.google.com/privacy

Can you opt out? Yes. You can opt out of Google Analytics tracking by:

  • Installing the Google Analytics Opt-Out Browser Add-On
  • Adjusting your browser’s cookie settings to block third-party cookies
  • Using the opt-out mechanism in our cookie consent banner
  • Submitting a “Do Not Sell or Share My Personal Information” request (see Section below)

3. Functionality Cookies

What they do: These cookies allow the Sites to remember choices you make and preferences you set in order to provide a more personalized experience. For example, they may remember your language preferences or your location for provider search purposes.

Can you opt out? Yes, through your browser settings or our cookie consent banner. Note that disabling functionality cookies may affect the performance of certain Site features such as provider search.

4. Location-Based Services (Google Places)

What they do: We use Google Places, a service operated by Google LLC, to power our provider search and location-finding features. When you use the provider search on our Sites, Google Places may collect information about your device’s approximate location (with your permission) and your search inputs in order to return geographically relevant results.

Who sets them: Google LLC (third-party)

Google’s Privacy Policy: https://policies.google.com/privacy

Can you opt out? You can disable location access for our Sites through your browser or device settings. Disabling location access will not prevent you from using provider search — it will simply prevent location-based pre-filling of search fields.

5. Behavioral Analytics and Session Recording (Microsoft Clarity)

What they do: We use Microsoft Clarity, a behavior analytics service provided by Microsoft Corporation, to understand how visitors interact with our Sites. Clarity collects anonymized data including mouse movements, clicks, scrolling behavior, and session recordings (visual replays of user sessions on our public pages). This information helps us identify usability issues, improve navigation, and enhance the overall experience on our Sites.

Who sets them: Microsoft Corporation (third-party)

Important: Clarity is only active on the public-facing portions of our Sites. It is not used on the Patient Portal, appointment scheduling pages, intake forms, or any page where protected health information (PHI) may be entered. Clarity is configured to automatically mask all text inputs, preventing the collection of any information you type into forms on our Sites.

Data collected: Anonymized/pseudonymous interaction data including mouse movement, click and scroll patterns, session recordings, heatmap data, and device/browser information. Clarity does not collect PHI.

Microsoft’s Privacy Statement: https://privacy.microsoft.com/en-us/privacystatement

Can you opt out? Yes. You can opt out through our cookie consent banner when you first visit our Sites, or by adjusting your browser’s cookie settings to block third-party cookies.

When you first visit our Sites, you will be presented with a cookie consent banner asking for your consent to place non-essential cookies (analytics, functionality, and location cookies) on your device. You may:

  • Accept all — allow all cookie categories
  • Reject non-essential — allow only strictly necessary cookies
  • Manage preferences — choose which categories of cookies you accept

Browser Controls

Most browsers allow you to control cookies through their settings. You can:

  • View and delete individual cookies
  • Block all cookies
  • Block third-party cookies specifically
  • Set your browser to notify you when a cookie is being placed

Please note that blocking all cookies may significantly affect the functionality of our Sites and other websites you visit. For instructions specific to your browser, consult its help documentation:

Global Privacy Control (GPC)

PromiseCare honors the Global Privacy Control (GPC) signal. If your browser or browser extension transmits a GPC signal when you visit our Sites, we will treat it as a valid opt-out of the sale or sharing of your personal information under California law (CPRA § 1798.135).

Do Not Track (DNT)

Our Sites do not currently respond to Do Not Track (DNT) signals from browsers. We rely on our cookie consent banner and GPC signal recognition as our primary mechanisms for honoring user opt-out preferences.

California Privacy Rights

If you are a California resident, your personal information collected through cookies is subject to the rights described in our Privacy Policy, including the right to know, the right to delete, the right to correct, and the right to opt out of the sale or sharing of your personal information.

To opt out of the sale or sharing of your personal information collected through cookies, click the Do Not Sell or Share My Personal Information link in the footer of our Sites, or contact us using the information below.

Cookies and Protected Health Information

PromiseCare is a HIPAA-covered entity. The cookies and tracking technologies described in this policy operate only on the public-facing portions of our Sites and do not collect, process, or transmit protected health information (PHI). The Patient Portal is operated through a separate, secure platform and is not subject to this Cookie Policy.

If you have concerns about the privacy of your health records or PHI, please review our HIPAA Notice of Privacy Practices.

We may update this Cookie Policy from time to time to reflect changes in the technologies we use or in applicable law. When we make changes, we will update the “Last Updated” date at the top of this page. For material changes, we will display a prominent notice on the Sites. We encourage you to review this policy periodically.
