Please read these Terms and Conditions carefully before using the PromiseCare website. By accessing or using the PromiseCare Medical Group website at promisecare.com, promisecaremanagementservices.com, hemetcommunitymedicalgroup.com, and all affiliated PromiseCare websites (collectively, the “Sites”), you agree to be bound by these Terms. If you do not agree to these Terms, please do not use the Sites.
1. Who We Are
PromiseCare Medical Group is an Independent Physician Association (IPA) and healthcare network serving Riverside County, California. Our affiliated physician groups include Hemet Community Medical Group, Family Senior Medical Group, Temecula Valley Physicians Medical Group, and Menifee Valley Community Medical Group. We operate 60+ primary care providers, 400+ specialists, and 18 urgent care centers across the Inland Empire.
These Terms govern your use of our public-facing websites. They do not govern the clinical care relationship between you and any PromiseCare-affiliated provider — that relationship is governed by separate clinical agreements, consent forms, and applicable law.
2. Nature of the Sites
The Sites provide:
- Information about PromiseCare’s network of providers, specialties, services, urgent care centers, hospitals, and imaging centers
- Patient and member resources and health education content
- Access to the PromiseCare patient portal (operated by NextMD, a third-party service provider)
- Medicare plan guidance and enrollment-support information
- Newsletter and health education subscriptions
- Contact and inquiry forms for member services, enrollment, and general inquiries
The information on these Sites is for educational and informational purposes only. Use of these Sites does not create a physician-patient relationship, a provider-patient relationship, or any clinical or treatment relationship of any kind between you and PromiseCare Medical Group or any of its affiliated providers.
3. No Medical Advice
The health information, articles, and educational content on the Sites are not intended to be a substitute for professional medical advice, diagnosis, or treatment. Always seek the advice of your physician or other qualified health provider with any questions you may have regarding a medical condition. Never disregard professional medical advice or delay seeking it because of something you have read on the Sites.
In the event of a medical emergency, call 911 immediately. Do not use these Sites to seek emergency medical assistance.

4. Medicare and Insurance Plan Information
The Sites contain information about Medicare Advantage plans and health insurance options that include PromiseCare’s provider network. This information is provided to help current and prospective members understand their healthcare options and is subject to the following limitations:
- Plan benefits, premiums, cost-sharing, and network participation change annually and may change mid-year
- PromiseCare does not represent all Medicare Advantage plans available in your service area
- Nothing on the Sites constitutes a guarantee of coverage, plan benefits, or enrollment acceptance
- For a complete list of Medicare options available to you, visit Medicare.gov, call 1-800-MEDICARE (1-800-633-4227, TTY: 1-877-486-2048), or contact your local State Health Insurance Program (SHIP)
We do not offer every plan available in your area. Currently we represent 12 organizations which offer 69 products in your area. Please contact Medicare.gov, 1-800-MEDICARE, or your local State Health Insurance Program (SHIP) to get information on all of your options.
5. Patient Portal
Access to the PromiseCare Patient Portal is operated by NextMD and is subject to additional terms and conditions established by NextMD and PromiseCare. You are responsible for:
- Maintaining the confidentiality of your login credentials
- All activity that occurs under your portal account
- Notifying PromiseCare immediately if you suspect unauthorized access to your account
The Patient Portal transmits sensitive health information. Do not access the Patient Portal from unsecured or public networks. PromiseCare is not responsible for unauthorized access resulting from your failure to maintain account security.
6. Contact Forms and Communications
The contact forms on these Sites are intended for general inquiries only. The forms are explicitly not designed for the transmission of protected health information (PHI). Do not submit medical records, diagnoses, test results, medications, or any other PHI through the Sites’ general contact forms.
By submitting a contact form and selecting “Yes” to receive text or email communications, you consent to receiving communications from PromiseCare as described in our Privacy Policy and SMS & Email Communications Policy. Consent to receive marketing communications is not required as a condition of receiving medical services from PromiseCare.
7. Acceptable Use
You agree to use the Sites only for lawful purposes and in a manner that does not infringe the rights of others. You agree not to:
- Use the Sites in any way that violates applicable federal, California, or local law or regulation
- Transmit any unsolicited commercial communications through the Sites
- Attempt to gain unauthorized access to any portion of the Sites or any system or network connected to the Sites
- Use any automated means (bots, scrapers, crawlers) to access, collect, or index content from the Sites without PromiseCare’s prior written consent
- Impersonate any person or entity or misrepresent your affiliation with any person or entity
- Transmit any content that is defamatory, obscene, or otherwise objectionable
- Submit PHI or sensitive health information through general contact forms
8. Third-Party Links and Services
The Sites contain links to third-party websites and services, including insurance carrier sites, Quest Diagnostics locations, the NextMD patient portal, government health resources, and other external sites. These links are provided for convenience only.
PromiseCare does not control, endorse, or assume responsibility for the content, privacy practices, accuracy, or availability of any third-party website. Accessing a third-party site through a link on our Sites is at your own risk. PromiseCare is not a party to and is not responsible for any transactions, data exchanges, or interactions between you and any third-party provider.
9. Intellectual Property
All content on the Sites — including but not limited to text, articles, health education materials, provider directories, logos, graphics, photography, and the overall design and compilation of the Sites — is the property of PromiseCare Medical Group or its content licensors and is protected by United States and California copyright, trademark, and other intellectual property laws.
You may access and use Site content for your personal, non-commercial informational use only. You may not:
- Reproduce, distribute, or publicly display Site content without prior written permission from PromiseCare Medical Group
- Modify, adapt, or create derivative works from Site content
- Use PromiseCare trademarks, logos, or brand elements without express written authorization
- Remove or obscure any copyright, trademark, or other proprietary notices

10. Privacy and Health Information
Your use of the Sites is governed by our Privacy Policy. For information about how PromiseCare handles your protected health information as a HIPAA-covered entity, please review our HIPAA Notice of Privacy Practices.
11. Disclaimers
THE SITES AND ALL CONTENT, INFORMATION, AND SERVICES PROVIDED THROUGH THE SITES ARE PROVIDED ON AN “AS IS” AND “AS AVAILABLE” BASIS WITHOUT WARRANTIES OF ANY KIND, EITHER EXPRESS OR IMPLIED, INCLUDING BUT NOT LIMITED TO IMPLIED WARRANTIES OF MERCHANTABILITY, FITNESS FOR A PARTICULAR PURPOSE, OR NON-INFRINGEMENT.
PromiseCare Medical Group does not warrant that:
- The Sites will be available, uninterrupted, or error-free
- Defects will be corrected
- The Sites or the servers that make them available are free of viruses or other harmful components
- The information on the Sites is complete, accurate, reliable, or current
12. Limitation of Liability
TO THE FULLEST EXTENT PERMITTED BY APPLICABLE LAW, PROMISECARE MEDICAL GROUP AND ITS AFFILIATED PHYSICIAN GROUPS, DIRECTORS, OFFICERS, EMPLOYEES, AGENTS, AND CONTRACTORS SHALL NOT BE LIABLE FOR ANY INDIRECT, INCIDENTAL, SPECIAL, CONSEQUENTIAL, OR PUNITIVE DAMAGES — INCLUDING LOST PROFITS, LOSS OF DATA, PERSONAL INJURY, OR PROPERTY DAMAGE — ARISING OUT OF OR IN CONNECTION WITH YOUR USE OF, OR INABILITY TO USE, THE SITES OR THEIR CONTENT, EVEN IF ADVISED OF THE POSSIBILITY OF SUCH DAMAGES.
IN NO EVENT SHALL PROMISECARE MEDICAL GROUP’S TOTAL LIABILITY TO YOU FOR ALL CLAIMS ARISING OUT OF OR RELATED TO THESE TERMS OR YOUR USE OF THE SITES EXCEED ONE HUNDRED DOLLARS ($100).
Some jurisdictions do not allow the exclusion or limitation of certain warranties or damages. In such jurisdictions, the above limitations apply to the fullest extent permitted by law.
13. Indemnification
You agree to indemnify, defend, and hold harmless PromiseCare Medical Group, its affiliated physician groups, and their respective directors, officers, employees, and agents from and against any claims, liabilities, damages, losses, and expenses — including reasonable attorneys’ fees — arising out of or in any way connected with your access to or use of the Sites, your violation of these Terms, or your violation of any third-party rights.
14. Governing Law and Dispute Resolution
These Terms and your use of the Sites are governed by the laws of the State of California, without regard to its conflict of law provisions. Any dispute arising out of or relating to these Terms or the Sites shall be resolved exclusively in the state or federal courts located in Riverside County, California, and you consent to the personal jurisdiction of such courts.
15. Changes to These Terms
PromiseCare Medical Group reserves the right to modify these Terms at any time. Changes will be effective upon posting to the Sites, and the “Last Updated” date at the top of this page will be revised accordingly. Your continued use of the Sites after the posting of revised Terms constitutes your acceptance of the changes. We encourage you to review these Terms periodically.
