About the Medicare Plan Information on This Site

PromiseCare Medical Group is a physician association network (IPA) serving Riverside County and the surrounding Inland Empire. Our network of primary care providers, specialists, urgent care centers, and partner hospitals participates in a number of Medicare Advantage plans. We provide Medicare plan information on this Site to help current and prospective members understand which plans include access to our provider network.

This Site does not enroll you in a Medicare plan. Enrollment in a Medicare Advantage plan must be completed directly with the plan sponsor or through a licensed Medicare enrollment agent. To explore all available Medicare options in your area, visit Medicare.gov or call 1-800-MEDICARE.

Important Plan Information Disclosures

Plan benefits change annually. Medicare Advantage plan benefits, premiums, cost-sharing (deductibles, copayments, coinsurance), provider networks, and formularies are established annually by each plan sponsor and approved by the Centers for Medicare & Medicaid Services (CMS). Benefits available in one contract year may not be available in subsequent years.

Network participation is subject to change. The inclusion of PromiseCare’s provider network in a Medicare Advantage plan can change from year to year. Before enrolling in or renewing a plan, confirm that your preferred PromiseCare providers are still in-network for the coming plan year.

Nothing on this Site constitutes a guarantee of coverage, benefits, or enrollment. Enrollment in a Medicare Advantage plan is subject to the applicant meeting applicable eligibility requirements and the plan sponsor’s acceptance of the enrollment application.

Enrollment periods apply. You may generally only enroll in, change, or disenroll from a Medicare Advantage plan during designated enrollment periods, including the Annual Enrollment Period (October 15 – December 7), the Medicare Advantage Open Enrollment Period (January 1 – March 31), and Special Enrollment Periods triggered by qualifying life events.
